Accomplishments
2004
January: Received the March of Dimes 2004 Community Grant to develop and implement a smoking cessation Initiative. The initiative is titled, "Baby Don't Smoke" A Smoking Cessation during Pregnancy Initiative. The Bronx Health Link, Inc./Perinatal Information Network was the only communty based organization in the Bronx to be awarded funding under this grant during 2004. The intitiative provides enhanced outreach worker training to the Bronx Based Community Health Worker and Healthy Fmmilies Programs. These workers in turn provide education and support for smokers who are pregnant or porpartum with a child up to one year of age. Additional support is provided to smokers residing within their households.
May: 2nd Annual Bronx-wide Baby Safety Shower. This health event provides community information and education around those areas which increase the risk of poor birth outcomes and infant deaths. This year over twenty Bronx Chase Banks participated by displaying educational literature in the lobbies for consumers and personnel. In addition to the kick-off event, several organizations held their own Safety Showers at their locations.
2003
March: Covering The Uninsured Week-The Bronx Health Link, Inc. coordinated a Bronx Town Hall meeting in collaboration with the Office of the Bronx Borough President and various organizations to bring attention to the plight of the vast number of uninsured individuals in New York City.
June: Community Health Education Day-The Bronx Health Link, Inc. in collaboration with the Office of the Bronx Borough President and Montefiore Medical Center, coordinated the Bronx's signature health event. Over one hundred providers conducted outreach and education at this event and over 700 attendees benefited from the day's activities.
October: The Bronx Health Link, Inc. helds its first Health Literacy Forum to increase awareness of the issues surrounding Health Literacy and the impact is has on the delivery and access of health care and social services.
AWARDS
2001
The Association of Hispanic Healthcare Executive recognized TBHL for its exemplary achievement and executive leadership on the delivery of healthcare services to Latinos and the diverse communities of NYC.
1999
The Bronx Health Link, Inc. won a national honor for the Bronx County. The Crown Community Award acknowledges the collaborative model and our accomplishments from 1998 to 1999. Over 70 nominations were received, with only 9 projects awards. The Bronx Health Link was the only health project recognized.
GRANTS
CPPSN (PIN)
In June 2001, The Bronx Health Link was the recipient of the New York State Department of Health’s Comprehensive Prenatal Perinatal Services Network grant, giving TBHL the distinguished honor of serving as the New York State recognized Perinatal Information Network (PIN) in the Bronx. The mission of the PIN is to improve the maternal-child health issues in the Bronx through collaborations, networking, and resource building, and by providing workshops and technical assistance to local perinatal providers and community residents
Infant Mortality Initiative
In September 2001, TBHL received funding from the NYC Department of Health for an Infant Mortality Initiative. This initiative provided the opportunity for promoting and supporting services to reduce the rate of infant mortality.
Bronx Coordinator for the NYC Public Health Partnership
In 2002, TBHL was awarded a grant to assume the role of the Bronx Coordinator for the New York City Public Health Partnership. This provides the opportunity for TBHL to enable the organizations and community stakeholders in the Bronx to participate in a citywide initiative. TBHL, as the Bronx Coordinator, is committed to coordinate efforts of Bronx providers and community stakeholders, provide assistance to providers, identify available resources, and maintain a watch on emerging community health issues.
March of Dimes
In June 2002, TBHL received a grant award from the March of Dimes to develop and implement mini-Infant Mortality Resource Centers located throughout the 12 community boards in the Bronx. These sites include health care facilities, high schools, colleges and local agencies.
