Community Health Worker Programs (Programas de trabajador de salud communitario)

The Community Health Worker Program is a home visiting program that serves pregnant women and families with infants up to the age of one year old. Anyone with a low income is eligible, regardless of immigration status. If you do not live in the
zip code covered by the agency, program staff can refer you to another agency.
Families receive education on a variety of topics, including prenatal care, domestic violence, family planning and breastfeeding. Community Health Workers also provide referrals and follow-up to other needed services.
